With the Everfeet Orthotics Protection Plan, you can have peace of mind knowing your new orthotic devices are fully covered against wear, breakage, loss, theft, and irreparable damage. Our Complete Protection Plan goes the extra mile by also offering outgrowth coverage for a two-year period.

One adjustment and/or refurbishment of your orthotics as originally prescribed by your physician for a period of two years.
Replacement coverage of devices is not included.

Two adjustments and/or refurbishments for a period of two years. Or, one replacement of pediatric outgrowth, lost, stolen, broken or irreparably damaged devices for a period of two years.
- A surcharge of $60 will apply for lost and stolen devices.
- A surcharge of $60 will apply for outgrown devices. (New impressions needed for outgrown devices)
The warranty is an agreement between Everfeet Orthotics and the patient, or the patient's parent, that only covers the cost of the device. The patient is responsible for working with the physician to process the warranty claim, and any fees charged by the physician are not covered by the warranty. The physician should only bill the patient or their insurance carrier for services such as evaluation and management, or any scanning or casting fees that may be required, but not for the actual device.

Terms & Conditions
I acknowledge and agree to the terms and conditions stated in the description of the Everfeet Orthotics Protection Plan. I understand that this program can only be obtained through my prescribing practitioner's office, and once purchased, it is non-refundable and non-transferable.
- To be eligible for coverage, enrollment in this plan must take place within 90 days of the orthotic devices being shipped by the lab. Coverage will commence on the date that the devices were shipped to the practitioner's office and will remain in effect for a duration of two years.
- The plan does not include coverage for any fees charged by your practitioner for casting or office visits. You will be responsible for paying these fees.
- Return shipping charges may apply.
- To initiate a claim, it is mandatory for you to first consult with your practitioner. At Everfeet Orthotics, we cannot directly act on patient requests, and we need a prescription from your practitioner before we start working on your devices.
- The surcharge of $60 under the Complete Protection Plan is applicable in cases where the laboratory cannot receive the original devices.
